HCCI Workshops Bring Ongoing Support and
Will Be Offered In-Person and Virtually
One of the best parts of HCCI’s Essential Elements of Home-Based Primary Care and Advanced Applications of Home-Based Primary Care workshops isn’t on the syllabus. “The workshop lays the foundation, but then also connects you to a caring faculty that wants to see each one of our learners thrive,” says Jeannine Lyons, HCCI’s director of education.

“There just aren't enough hours in the day to do the clinical work as well as program development for those who are solo trying to create successful HBPC practices, ”says workshop attendee Nichole Bateman, PA-C, MPAS, of Integris Mobile Care in Miami, Oklahoma. “Having access to HCCI's guidance and materials has been great. I look forward to networking with other program participants and will be reaching out to HCCI staff, too.”

The next HCCI workshops, first Essential Elements of HBPC in Orlando from December 2-3, then Advanced Applications of HBPC in Schaumburg, IL, from December 9-10, mark a return to in-person meetings for HCCI (subject to status of the pandemic). Though HCCI is returning to in-person meetings, the organization’s adapted virtual courses had such high engagement and participation this year they will add virtual options for some 2022 offerings.
How One Practice Is Reframing Palliative Care
One big challenge for Lightways Hospice and Serious Illness Care, an independent, nonprofit Joliet, Illinois-based practice that serves 250 seriously ill patients in their homes, was reframing serious illness and palliative care.

“Serious illness care is a comprehensive, long-term service for patients facing serious and chronic illness — it’s not just for end-stage patients who don’t want hospice,” says Sara Dado, LCSW, Senior Director of Clinical Programs for Lightways. “We're always faced with the challenge of educating our community, patients, and even our staff, on what palliative care is… and what serious illness care is intended to be. To overcome this, we continue to talk about what we do, and think about the services we provide, in a different way.”

Updated Online Course Offers Strategies for Demonstrating HBPC’s Value
As an HBPC provider, if you’re working in a fee-for-service (FFS) model, you bill each time you provide a service without the opportunity to be paid on quality outcomes. Therefore, the financial resources to enhance your patients’ quality of life may be limited. Value-based payment arrangements are increasingly common in the care of homebound, medically complex patients, and the prevalence of payer contracts recognizing the value of HBPC has been a game changer. This recently updated on-demand course will help you demonstrate the value of HBPC when negotiating with payers to obtain necessary funding to be more supportive of your patients’ needs and to maintain financial sustainability for your practice.
