HCCI House Call Practicum™
is a unique two-day field experience that allows providers and practice managers to shadow trained preceptors and acquire new knowledge and skills in house call operations and clinical care for medically complex patients in the home. The HCCI House Call Practicum™ offers an individualized learning opportunity at Cleveland Clinic, one of our Practice Excellence Partner (PEP)™ sites, programs recognized nationwide as premier providers of HBPC.
You may also wish to add the optional Telemedicine Elective as a third day or participate only in the Telemedicine Elective! This full-day activity is designed to familiarize you with the technology and staffing model used to conduct virtual visits.

HCCI will present a Preconference at the Gerontological Advanced Practice Nurses Association (GAPNA) Annual Conference on Wednesday, October 2, 12 pm - 5 pm, in Las Vegas, NV. This in-depth session,
House Calls: APNs Navigating Challenges & Implementing Solutions,
will be taught by national NP leaders and will explore the challenges faced and opportunities presented when caring for medically complex patients in the home. Learners will come away with new insights into NP-driven practices, time efficiencies, documentation and coding for house calls, valuing your practice, payer/ACO partnerships, and community resources. This two-day classroom workshop is designed for experienced HBPC providers. Case-based sessions focus on higher-level topics in HBPC such as advance care planning, prognostication, care coordination and transitions of care, caregiver support, and challenging clinical issues such as dementia, polypharmacy, selected procedures and wound care.
New at the December course are hands-on sessions demonstrating wound care, gastrostomy and tracheostomy tube changes, and joint injections. The ability to automate and/or optimize routing of patient visits is key to minimizing travel time for providers, boosting productivity and increasing face-to-face time with patients.
