It's February and HCCI is Celebrating Heart Month!
We love house calls. It's Valentine's Day, a day all about love, and we're sharing our love for a remarkable, unique model of care. House calls allow us to connect deeply with patients and family members. House calls allow us to make a lasting impact on patients who are in real need of meaningful care. House calls enable us to achieve health care's Triple Aim and bring needed change to our nation's healthcare landscape. Tom Cornwell, M.D.
Tom Cornwell is the CEO of Home Centered Care Institute (HCCI) and past president of the American Academy of Home Care Medicine. He's been a house call doctor for more than 25 years and made more than 33,000 house calls. Tom has received extensive local and national media attention for his work including national PBS TV and TIME Magazine features.

Gresham Bayne, M.D.
Gresham Bayne is considered the father of the modern day house call. He founded The Call Doctor Medical Group in 1985 and serves as Executive Medical Director. He has presented the mobile healthcare concept in scores of national publications and in keynote speeches for the AMA.

K. Eric De Jonge, M.D.
Eric De Jonge is the Executive Director of MedStar House Call Program and Director of Geriatrics, MedStar Washington
Hospital Center. Eric is the President of the American Academy of Home Care Medicine, serves as a faculy member for HCCI and is an advocate for HBPC in Washington D.C.

Elizabeth McCormick, M.D.
Elizabeth McCormick is Assistant Professor, Geriatrics, Palliative Medicine,
Internal Medicine, Icahn School of Medicine at Mount Sinai and Associate Director, Mount Sinai Visiting Doctors
Program/Chelsea Village House Call Program.


Paul Chiang, M.D.
Paul Chiang is the Chief Medical Officer for the Home Centered Care Institute (HCCI)
and Medical Director for Northwestern Medicine HomeCare Physicians. Paul has been a practicing house call physician for 19 years.

Ron Ordona, DNP, FNP-BC and Rachel Zimmer, DNP, AGPCNP
Ron Ordona is a Nurse Practitioner with with Senior Care Clinic in Northern California specializing in house call medicine. Rachel Zimmer is Assistant Professor, Gerontology and Geriatric Medicine at Wake Forest School of Medicine and a Nurse Practitioner with Wake Forest Baptist Health.
