Volume 3 Issue 2 | February 2019
It's February and HCCI is Celebrating Heart Month!
We love house calls. It's Valentine's Day, a day all about love, and we're sharing our love for a remarkable, unique model of care. House calls allow us to connect deeply with patients and family members. House calls allow us to make a lasting impact on patients who are in real need of meaningful care. House calls enable us to achieve health care's Triple Aim and bring needed change to our nation's healthcare landscape. Whether you're a provider, a practice manager, a payer, a policymaker or a patient, take a moment and feel the love.

Our new video series, I "Heart" House Calls features house call providers and patients and their stories of how house calls have permanently, and wonderfully, affected their lives. Do you have a love story to tell about house calls? Please contact us. We'd like to share your story.
