We know nurse practitioners (NPs) are playing an ever-expanding role in the delivery of HBPC, and HCCI wants to engage with and support these NPs.

AANP Corporate Council

AANP National Conference
in Indianapolis, June 18 - 21. HCCI will also present a Pre-conference at the Gerontological Advanced Practice Nurses Association (GAPNA) Annual Conference, on Wednesday, October 2, 12-5pm, in Las Vegas, NV. This in-depth session,

will be taught by national NP leaders and will explore the challenges faced and opportunities presented when caring for complex patients in the home. , featuring HCCI’s CEO, Tom Cornwell, MD, who was joined by Mark Grzeskowiak, MD, HDMC, Chief Medical Officer of JourneyCare,
Illinois’ largest nonprofit for palliative, supportive and hospice care.
In 2018, HCCI initiated a partnership with JourneyCare to expand access to home-based primary care for patients across the Chicagoland area. In this webinar, Drs. Cornwell and
Grzeskowiak discussed the intersection of home-based primary care and home-based palliative care, and they shared with the audience the successes and key learnings from the first year of the HCCI/JourneyCare pilot program. The webinar was well received and attracted a record-high number of participants.
