HCCI Team Members Receive Prestigious
American Academy of Home Care Medicine Awards

On Friday night, October 18th, during its annual meeting in Rosemont, IL, the American Academy of Home Care Medicine (AAHCM) recognized two team members of HCCI for their outstanding contributions to the field.
- Paul Chiang, MD, medical director and practicing physician of Northwestern Medicine’s HomeCare Physicians and senior medical & practice advisor for HCCI, was awarded AAHCM's House Call Physician of the Year.
- Aaron Yao, PhD, HCCI's lead researcher, was awarded AAHCM's Dan Gilden Creative Investigator designation.
“We’re very proud of Dr. Chiang and Dr. Yao,” said Dr. Thomas Cornwell, CEO of HCCI, founder of Northwestern Medicine’s HomeCare Physicians and a practicing house call doctor. “Together, they help us spread awareness about home-based primary care and build a workforce of providers and practices to make this model of care more accessible. In doing so, they bring a real commitment and caring to their work.”
“These awards represent the highest professional honor from our organization, selected by a committee of peers,” said Theresa Soriano, MD, MPH, president of AAHCM. “We are thrilled to be able to recognize and celebrate Dr. Chiang and Dr. Yao, who embody the dedication, advocacy and idealism that have helped our field grow and thrive over the last thirty years.”

HCCI Drives Learning and Sharing
at the AAHCM Preconference
|
|
On Thursday, October 17th, over 130 providers, practice leaders and operations staff, and others attended the AAHCM Preconference hosted by the Home Centered Care Institute (HCCI) in Rosemont, IL, which kicked off the annual meeting of the American Academy of Home Care Medicine.
Entitled
House Calls: Achieving Clinical Excellence and Sustainability
,
the Preconference, funded in part by a grant from The John A. Hartford Foundation, was designed to help participants enhance their medical knowledge, patient care, and interpersonal and communication skills. The day-long session spotlighted the complex care needs of homebound patients with serious illness, and their caregivers, by weaving together a case-based discussion, mini-lectures and a Q&A session. It also included information on coding and billing, increasing efficiencies in practice operations and creating a value proposition to tell a practice’s story to payers, ACOs, and health systems.

“All those who provide care and comfort to people in their homes are privileged to witness the many ways in which ‘giving’ is expressed through families, caregivers, colleagues, and patients themselves,” offers Heather Hutchison, Chief Development Officer of the Home Centered Care Institute (HCCI).
As the holiday season approaches, we will all be asked to give through our time and talents — and financially. GivingTuesday, a global day of giving, helps to remind us that we can give to a purpose, a cause, that reaches well beyond ourselves, like home-based primary care (HBPC). HBPC’s purpose is to care for those who are homebound or home-limited and this cause, by its very name, begins at home.

CMS Announces Request for Applications
for Primary Care First Model Options
|

CMS released the
Request for Applications (RFA)
for the Primary Care First (PCF) and Seriously Ill Population (SIP) models Thursday, October 24. The practice application period also started that day while the
deadline to apply via the
live portal
is January 22, 2020
.
The model will now begin in January 2021 (a delay of one year from the originally anticipated start date). CMS will select practices and payers during Winter/Spring 2020 and will onboard participating practices and payer partners to the model from July to December 2020.
