HCCI participates in “special” C-TAC
National Summit on Advanced Illness Care

There was something special about the
National Summit on Advanced Illness Care held October 9-11 in Minneapolis, as the participating members of the
Home Centered Care Institute (HCCI) quickly learned. That “something” was a mixture of collaboration, a continuous improvement mindset and a passion for the patient. A passion found in the meeting’s theme of “Lead. Innovate. Inspire.”
The summit, hosted by the
Coalition to Transform Advanced Care (C-TAC), and sponsored by several organizations, including HCCI, brought together hundreds of providers, practices, organizations, innovators and thought leaders. Over the three days, the participants took part in presentations and panels, interactive “labs,” breakout sessions, exhibits and countless opportunities for networking.
Information shared ranged from innovations and trends in advanced care to recent changes in the public policy environment with Medicare’s new serious illness payment models. In addition, there was a clear focus on how the healthcare industry, community and faith-based organizations, government officials, activists, and other key partners are coming together to create a more holistic “person-centered” care model, where patients actively participate in their own medical treatment.

Practice Management Tip of the Month
Demonstrating the value of your HBPC practice through data
|
|
Home-Based Primary Care (HBPC) practices must have the ability to tell the story of the care provided. They can do that by spotlighting the data linked to their high-quality outcomes. While success may look different from one practice to another, there are commonalities in how HBPC practices are evaluated across the field.
Is your practice currently tracking and reporting meaningful clinical quality metrics and practice characteristics that demonstrate the value of the care you provide? If not, you can work with your analytics and EHR team to optimize the reporting capabilities and evaluation plan for your practice. Below are a few examples of meaningful metrics that reinforce the importance of HBPC:
Utilization Metrics
- Hospital admissions and readmission rate and main diagnosis for each admission
- Reduction in acute care utilization (i.e., ED visits and hospitalizations 6-12 months before/after HBPC)
- Total number of deaths at home vs. hospital or SNF
- Total number of deaths on hospice, hospice length of stay
Practice Characteristics
- Average visits per day/week/month per provider, per place of service (home vs. assisted living), and for the practice
- Number of new referrals per month and a running total of active empaneled patient census
- Billing code breakdown by provider and practice as a whole (e.g., Bell curve reports)
- Average mileage per day by provider and practice
- Patient and caregiver satisfaction
- Time from referral to first visit
Data can demonstrate a more holistic view of the overall improvement in your practice's health outcomes and cost savings than looking just at your bottom line. Telling your story with data will help your practice become marketable to potential partners and value-based arrangements. Be sure to identify and report meaningful metrics so you can show the true value of your practice.
