HEALTHY YOU Program Updates

For those who will be on the Hajoca Medical plan in 2017:

Instead of starting at a higher premium and using participation in Healthy You to reduce medical insurance premiums (the amount deducted from your paycheck bi-weekly for the cost of medical insurance), we will now be starting at a lower premium and participation in Healthy You will keep you at the lower premium. Non-participation in Healthy You will incur a penalty and increase your premium. We expect premiums to increase based on normal factors, such as medical trends and our experience, but an employee who fully participates in Healthy You this year should not expect a substantial increase.

By participating in the Healthy You program, you and your spouse
(if applicable) will have the opportunity to pay the lowest healthcare premium.

TOTAL POSSIBLE PENALTIES:

EMPLOYEE:        $50/check ($1,300 annually)

SPOUSE:             $50/check ($1,300 annually)

BOTH EE & SP   $100/check ($2,600 annually)

 
Program Requirements:
Eligible employees should have recently received, or will soon be receiving, a packet in the mail outlining the new Healthy You program details. The packet includes the Healthy You Program Launch Flyer, Healthy You Website Login Instructions, and two Health Screening Forms for you and your spouse (if applicable).

Tobacco-Free Affadavit

Participants must attest that they are tobacco-free by signing the affidavit on the Healthy You website.

Health 
Assessments

Online Assessment:
Complete the online Health Assessment through the Healthy You website.

Biometrics:
Schedule your FREE ANNUAL PHYSICAL EXAM (each plan member is entitled to one free exam per year) and ask your doctor to complete your Health Screening Form. You will then submit the completed form to Asset Health. 

Online Assessment:
If biometric numbers fall out of range, participants must complete the corresponding course on the Healthy You website.


PENALTY IF NOT SUBMITTED AND/OR COMPLETED BY NOVEMBER 30, 2016:

$25/Paycheck

Employees should access their Healthy You web account by using Internet Explorer to navigate to Hajoca.net. Find the Healthy You link:

SPOUSE* & MANUAL EMPLOYEE LOGIN:

Spouses should log into their accounts (and employees can manually access their accounts) by visiting:

Username: first initial + last name - all lowercase, no spaces
Password:
 Birthdate - MMDDYYYY

* IMPORTANT NOTE: All spouse data existing prior to May 23, 2016 will be migrated to new accounts. Previous login credentials will no longer provide access to the Healthy You site. Please use the listed credentials to access your new account. Your historical health data will be transferred to your new account within a few months.
