Recently added services for participants
|
In 2018, HEALTHeLINK started offering several new services to participants including:
- Access to 42 CFR Part 2 Data - This year we began receiving data from federally funded substance abuse facilities. Access to data coming from these facilities is granted based on a separate access request and is based on treatment related use cases only.
- Batch Alerts - Notifications to a practice of their patient's emergency room visit or hospital admission/discharge can now be delivered for all their patients in one file from the previous 24 hours, as well as individually at the time the admission and/or discharge happens.
- Self Service Authorized Contact (AC) Portal - Request access for users, manage users, or de-activate user rights through a self-service portal, available now for practice Authorized Contacts through the HEALTHeCOMMUNITY Portal, the same access point for HEALTHeLINK and HEALTHeNET.

New HEALTHeNET System Launched
|
Earlier this month we launched a new version of the HEALTHeNET system. The updated system was custom built for our community using state-of-the-art web technologies and it incorporates industry best practices for usability, scalability, security and privacy. Jackie Welsch co-chairs SHIEC committee
|
HEALTHeLINK Controller Jackie Welsch was recently appointed co- chair of Strategic Health Information Exchange Collaborative (SHIEC)'s group purchasing committee. The committee facilitates discussions with vendors that could provide a benefit to SHIEC HIE members due to economies of scale.

In the past couple of months, HEALTHeLINK has been awarded more than $900,000 in grants to leverage its HIE technology in advancing population health and supporting regional value-based care initiatives. The grants include:
- A $325,000 Interoperability and Innovation grant from the New York eHealth Collaborative (NYeC) to combine clinical and claims data. This creates a regional unified repository to provide a more complete picture of a patient's health from all care settings and gives treating providers the information needed for outcome-based reporting and population health analytics in support of the Comprehensive Primary Care (CPC+) program.
- A State Innovation Model grant through New York State Department of Health and NYeC totaling $384,000 focused on supporting the development and delivery to provider organizations of New York State Patient Centered Medical Home (PCMH) scorecard measures.
- In support of the Delivery System Reform Incentive Payment Program (DSRIP) and Community Partners of WNY, HEALTHeLINK was awarded $196,000 as a subcontractor to the Value Network IPA (which includes behavioral health practices) to support the upload and access of patient care plans and alert notifications to primary care providers.

Medication reconciliation
|
A comprehensive evaluation of a patient's medication regimen at event transition of care is key to avoiding medication errors and assessing compliance and adherence patterns.
Esra Mustafa and Tera McIlwain, pharmacists from Millennium Collaborative Care, have developed a webinar to provide education on the importance of medication reconciliation and review strategies to institute, or improve this process in the clinical setting.
