We are pleased to announce a new feature within the HEALTHeLINK Portal: the Health-Related Social Needs (HRSN) data query. Created in collaboration with NYeC as part of the New York Health Equity Reform 1115 Waiver program, this powerful tool enables providers to seamlessly retrieve HRSN screening, assessment, and referral data. By streamlining access to vital social needs information, the HRSN data query enhances patient care and facilitates more comprehensive and effective healthcare.

As shared in the December newsletter, HEALTHeLINK is migrating to a new Health Information Exchange (HIE) platform as part of the SHIN-NY’s shared infrastructure initiative. By joining the Advancing Shared Infrastructure Consortium, which includes HealtheConnections, Rochester RHIO, Bronx RHIO, and Tech by Design, HEALTHeLINK will be the first to transition to the chosen product suite, Fusion. The HEALTHeLINK team is diligently working on this migration, which involves many working pieces, to ensure a smooth transition. This move is a critical step in enhancing our HIE capabilities, ensuring improved interoperability, and supporting better patient care.

As a reminder, it is strictly prohibited for an Authorized User to disclose to any other person any passwords and/or other security measures issued to the Authorized User (HEALTHeLINK Policy P03 § 3.2.D & Policy SP-007 § 3.2.1.3). This includes inadvertent disclosures where usernames and/or passwords are written down and improperly safeguarded.

Civitas is a national nonprofit collaborative of more than 170 member organizations dedicated to improving health through HIE, data use, and cross-sector collaboration.
