Community Health Dashboard Update

HEALTHeLINK is excited to announce the launch of our innovative Community Health Dashboard, “HEALTHeWNY,” a comprehensive, publicly accessible platform designed to provide real-time data and insights into the health status of our community. The HEALTHeWNY Dashboard, a first-of-its-kind tool, is designed to empower local health departments, community organizations, healthcare providers, and policymakers with the information they need to make informed decisions about health and wellness in our community. HEALTHeWNY is a user-friendly, interactive tool that allows users to explore data at a county and zip-code level, providing a detailed picture of health in our community. It is updated regularly with the most recent data available, ensuring users can access the most accurate and timely information.

The launch of the Community Health Dashboard is a significant step forward in our commitment to improving the health and well-being of our community.

Phase 3 winner for HEALTHeQUALITY

HEALTHeLINK was the Phase 3 third place winner out of more than 130 initial applicants of the Building Bridges to Better Health: A Primary Health Care Challenge sponsored by the Health Resources & Services Administration (HRSA). The goal of this multi-phase challenge was to enhance access and care coordination to achieve better health and reduce disparities utilizing technical assistance solutions.

To advance health equity and specifically support Federally Qualified Health Centers, HEALTHeLINK built HEALTHeQUALITY, an innovative solution that brings together clinical and health-related social needs data to enable the identification of gaps in care to assist Federally Qualified Health Centers (FQHC) and other health care organizations. HEALTHeLINK worked with The Chautauqua Center, Jericho Road Community Health Center, and Universal Primary Care to utilize HEALTHeQUALITY to bridge the gap between clinical care and social concerns that act as barriers to care.

HEALTHeLINK Earns NCQA Validation

HEALTHeLINK has earned the Validated Data Stream designation in the National Committee for Quality Assurance (NCQA) Data Aggregator Validation program.

The validation applies to standard supplemental data for use in Healthcare Effectiveness Data Information Set (HEDIS) reporting. No primary source verification is required for HEDIS reporting when health plans report data from Data Aggregator Validation validated sources.

Data streams that earn validation undergo a rigorous, end-to-end look at the quality and integrity of data and the procedures used to manage and safeguard it. From ingestion at primary sources through transmission to end users, verifies adherence to NCQA process, system and data standards.

HEALTHeLINK in the Community

Nurses’ Night Out

On October 11, the HEALTHeLINK team participated in “Nurses’ Night – Celebrating All Healthcare Heroes.” It was great to meet and greet with so many members of our health care community!

Informational Sessions

Earlier this fall, we hosted three information sessions for our population health community partners at Holiday Valley, Genesee Community College and Northland Workforce Training Center. We shared what we do and what we have accomplished overhaw the past year since Population Health Collaborative became a part of our organization.

Annual Holiday Donation

HEALTHeLINK employees donated a total of $1,495 to St. Luke’s Mission of Mercy for this holiday season.

Employee News

HEALTHeLINK recently announced a couple of new promotions on our team.

Tiffany Embry was promoted to senior account manager. She has been instrumental in the account management team over the past few years and has proven herself to be invaluable to the HEALTHeLINK team. Tiffany previously was account manager.

Sarah Wise was promoted to senior healthcare informatics analyst from healthcare informatics analyst. With HEALTHeLINK for more than three years, Sarah has proven to be an integral part of our reporting team.

GUIDE 2 BETTER CARE SEGMENTS

HEALTHeLINK’s Guide 2 Better Care segments air every other Wednesday on WGRZ-TV, during the 4pm and 11pm newscasts. Local health care leaders provide education about HEALTHeLINK as well as highlight the value and benefits of HIE in Western New York.
