A few updates since our last email on this topic:
HHS#4 applications are due by 10/26/2021. You are required to re-register when completing this application. Please be aware this is a multi-step process, you should not wait until the last day to apply.
We have been notified of the quarters and information required for the application. We have updated our worksheet to reflect the correct quarters. Please CLICK HERE.
If you apply, you will be required to report both patient collections and expenses for the requested quarters.
You can include the total (all) expenses, excluding doctor wages and benefits, for the quarter.
If you receive the money, you will only be allowed to use it on Dental Supplies, Wages & Labs. (Yes, they are using one method to determine funding and a different method on what money received can be used for when reporting).
Collections and expenses for the following quarters will be required to complete the application:
2019 Q1, 2019 Q3, 2019 Q4, 2020 Q3, 2020 Q4, 2021 Q1

To locate income required for the application:
- The Dental software for your practice can provide the collection numbers required. You will print the reports based on the dates for each quarter.
- The 12 month rolling statement provided with your December monthly reports from our office, will give you the expenses listed by month to use

- You will find this information on your profit and loss statement that was provided to you from our office.
- The 12 month rolling statement provided with your December monthly reports will give you the expenses listed by month to use.
