Latest news:
In November 2022, select types of Cardiology-related data from Joseph Brant Hospital were moved from either the Radiology and/or Transcriptions modules, to ClinicalConnect's Cardiology module. This means ClinicalConnect users can now find results for the following types of cardiac tests performed at Joseph Brant Hospital, including but not limited to the following, in the Cardiology module:
  • Echocardiograms (ECHOs)
  • Electrocardiograms (ECGs)
  • Cardiac Stress Tests
  • Holter Monitoring
  • Cardiac MRIs
  • Cardiac CTs

Please be sure to check the Radiology and/or Transcriptions module if you’re not seeing the data you’d expect to see in the Cardiology module. ClinicalConnect’s Data Integrations Chart provides a high-level summary of the data available in each clinical module. 

Helping Patients Empower
Their Health Journey
Since our last edition, a great deal of work has been undertaken to ready ConnectMyHealth for access by patients. Data from almost all existing SWO (Sunnybrook) MyChart hospital participants is now live in ConnectMyHealth, and advances have been made to enable patients to view the health records from provincially-managed repositories using ConnectMyHealth. However given the complexities of such an important project from a patient perspective, opening of account registration has been delayed, and the following updates were communicated to project stakeholders, as well as patients who have contacted us to express their interest in ConnectMyHealth. We wanted to share them in this newsletter also:

• Patients who have a SWO MyChart account can continue to have access to their data from participating regional hospitals via SWO MyChart until March 31, 2023 (extended from the previously-communicated December 31, 2022).
• Patients who did not already have a SWO MyChart account prior to the published September 14, 2022 registration cut-off date can continue to access their health records from the organizations where they have received care. 

We appreciate the importance for patients to have timely, electronic access to their medical information, and thank you all for your patience as we work to optimize the ConnectMyHealth registration and user experience. We'd also like to extend our thanks to Sunnybrook Health Sciences Centre for their collaboration in facilitating this data feed extension, and to our partners at Ontario Health, the Ministry of Health, Ontario Health West Region and its participating Ontario Health Teams, and data-contributing hospitals in this region for all their work and support for this project.

If you'd like to be notified by email when ConnectMyHealth account registration opens, please email [email protected] to be added to our distribution list. It's expected that registration will open before the end of March 2023.

Improving Access to
Specialist Advice
As part of the broader Ontario eServices Program, eConsult is a secure web-based tool that allows primary care providers timely access to specialist advice and often eliminates the need for an in-person specialist visit. eConsult has over 120 specialties and sub-specialties and is available to physicians and nurse practitioners free of charge. Using eConsult can improve care coordination and collaboration between clinicians and provide opportunities to manage patient cases at the family practice level, where patient trust is often highest.

1. Clinicians can now leverage their integrated EMR (TELUS PS Suite, OSCAR, or Accuro) to send an eConsult, through Ocean, using the same referral process used to submit Ocean eReferrals. Watch our training videos or see our Step-by-Step Guide to learn how to submit an eConsult through Ocean.
2. Coming Soon HRM® Integration for OTNhub: Ontario Health’s update to the OTNhub will allow those with Health Report Manager (HRM) access to export eConsult case details. HRM is a digital health solution that enables clinicians using an OntarioMD-certified EMR to securely receive patient reports electronically from participating hospitals and specialty clinics.

Tips & Tricks: It's As Easy as 1-2-3!
Attention Pharmacists! With the recent expanded scope of practice related to prescribing and renewing select medications, more than ever, ClinicalConnect provides Pharmacists with an efficient way to review their patients’ health records and gain additional insight into the patients' health history, to support their prescribing practices. We wanted to point out two important components that can help pharmacists optimize their user experience:

  1. ClinicalConnect's Dashboard: Pharmacists can leverage ClinicalConnect's Dashboard to optimize their use of the portal. When using ClinicalConnect on desktop devices, the "Dashboard" landing page is a great way to see the parts of the patient's health record that are most important to Pharmacists, in clinical modules such as Allergies, Pharmacy, Labs, Microbiology, Transcriptions - all on one screen. You can easily drag and drop the module icons from the Dashboard banner into the view, to populate the information that you find most important to your practice. You can also remove a module by clicking the Close button, and/or add a different one. Once configured, each time you access a patient’s record, the Dashboard will present information available for the patients according to the modules you added.
  2. "Days Back" Filter: Only seeing very recent data in ClinicalConnect for your patient? Be sure to check each module's preference settings by clicking the 'gear' icon, and check what the "Days Back" option is set to. This setting determines how much historical data you will see for your patient, and can be set individually for each clinical module, and is also accessible while using the Dashboard described above. If needed, increase the number of "days back", then click ‘Save and Apply’.

Meet Our Team
Name: Leia Patzernuk
Role: Transformational Specialist

Leia joined HHS’ HITS eHealth team almost eight years ago as a Transformational Specialist. For most of that time, Leia has been focused on leading our change management team who support healthcare service providers in south west Ontario adopt the regional digital health solutions. Since the summer, she’s been leveraging her many years of experience in quality management, service design, and information technology to help operationalize ConnectMyHealth.

Outside of work, Leia is the sitting Chair Elect for the Service Quality Division of the American Society for Quality (ASQ), soon to be assuming the role of Chair. She’s also an avid photographer. Since achieving her diploma in Professional Photography, she’s torn between traveling with her family to capture those beautiful landscapes and staying home to snuggle with her little dog Mojito.
