Hello Community Leadership,


This email is intended for supervisory staff and anyone else submitting requests for New HMIS Users for your agency. If this is not part of your responsibilities, please disregard.

Due to the high volume of emails and increasing number of users, we have been simplifying and improving the HMIS new user process. Not only have we simplified our forms, but we combined the original HMIS End User Agreement with the VI-SPDAT Agreement. We will continue to simplify this process for both our community partners and our department.  


As of August 1st, 2022, we will be requiring new user requests to conform to the following policies:


  • All documents submitted to HMIS must have wet signatures. We do not accept typed names in handwriting fonts as a signature on a legal document. If you are having trouble scanning documents, you can mail them directly to our office. Our address is in the message footer.


  • All documents must be filled out completely. If we receive incomplete documentation, we will require the documents be resubmitted.


  • To improve our ability to track your new user request, both PDFs must be sent in the same email to HMIS@SacStepsForward.org. If you can combine the PDFs into one file, we would be very grateful.



  • New user requests must be resolved within 30 days. If we have not heard from you or your new employee, we will send one email informing you of the date we will retire this request and remove the new user's name from our new user request list. To reinstate a request, simply fill out our online HMIS New User Request form again, which will put your new user back on our list.
