Message from the Nurse

Parents of 7th graders,
The State of Ohio requires all 7th grade students to have a T-dap vaccination and Meningitis vaccine to attend school. Review your shot records at home or call your physician to find out if your child is in need of these. Please turn in proof of shots by September 14, 2018. Your child can not attend school until proof is shown. Any absences due to not having these records will result in unexcused absences. At any time a parent/guardian can sign an exemption for vaccines and then get the vaccines at another time or if you do not wish for your child to receive these vaccinations you must have an exemption form on file and renewed each school year. These forms are available in the nurses office and need signed by a parent or guardian. If you would like your physician to fax your child's records, HMS fax number is 740-283-8949

The Jefferson County Health Department will offer immunization clinics Monday thru Thursday from 8-4 and Friday from 1-3. Please take a copy of your child’s shot record with you and a parent or legal guardian must accompany the child. The Health Department will bill certain insurance plans and Ohio Medicaid; otherwise there will be a $20.00 fee.
If you have any questions regarding this notice please contact the school nurse at Harding Middle School.
Thank you for your continued support of our schools.
