HOMERuN at SHM Converge 2023

The Hospital Medicine Reengineering Network (HOMERuN) is a national network of Hospital Medicine investigators at 12 academic medical centers (AMCs) and 50 affiliated sites.

HOMERuN convened meetings and social events among consortium members at SHM Converge 2023 to celebrate accomplishments and plan for future collaboration.

The Achieving Diagnostic Excellence through Prevention and Teamwork (ADEPT) Study seeks to create a learning health system which integrates diagnostic error and process assessments into existing quality and safety programs, shares data, infrastructure, and expertise to catalyze improvement in diagnostic performance, and lays the groundwork for efforts which will last beyond the duration of the grant.
Co-Investigator Katie Raffel along with Principal Investigators Jeff Schnipper and Andy Auerbach led our first in-person meeting with ADEPT colleagues across the 16 sites participating in the study to:

- Meet and introduce team members
- Propose titles for our NEJM publication in 2026
- Example: "Impact of a diagnostic error reduction program at 16 hospitals: Results of the ADEPT multicenter study"
- Propose titles for our NYT coverage
- Example: "Cracking the diagnostic code: ADEPT hospitalists tackle a previously unsolvable problem"
- Discuss how ADEPT teams might continue working together after the study ends

Annual Survey of Academic Hospital Medicine: Division Chiefs Meeting

The HOMERuN Executive Committee launched our first annual HOMERuN survey of Academic Hospital Medicine Division/Section Chiefs to compare and benchmark Academic Hospital Medicine Programs. Jeff Schnipper led our meeting of participants who completed the survey, joined by HOMERuN Executive Committee members, to:

- Discuss areas of confusion in the survey
- Discuss variations in hospitalist job descriptions
- Propose changes to the survey for the next year
- Review next steps:
- Correlate hospital medicine structures and processes with outcomes
- Benchmark results across sites

HOMERuN COVID-19 Collaborative Teams: SHM Teamwork Award
|
|
The SHM Award of Excellence in Teamwork in Quality Improvement is presented in recognition of exemplary quality improvement initiatives in hospital medicine that engage the full patient care team and intended to highlight outstanding team approaches to implementing interventions that improve patient care and safety.
HOMERuN COVID-19 Collaborative Work Groups, comprising of Discharge Planning, Workforce Management, Provider Wellness, Clinical Pathways, Venous Thromboembolism Prevention and Management, Medical Education, Patient Communication, and Antiracism Health Equity, and Justice as well as the Executive Committee were recognized with this award for:

- Producing wide-ranging and impactful support for hospitalists, hospitals, and patients and their family caregivers
- Exemplifying a nimble, scalable, near-real-time structure to share best practices and lessons for many challenges facing hospitalists and hospitals during the pandemic
- Connecting hospitalists across the US, rapidly sharing information to improve patient care
- Epitomizing dedicated collaboration among hospitalists, working to address the biggest challenge of our careers, together

Inter-Hospital Transfers Study Happy Hour
|
|
The Identification and Prevention of Potentially Inappropriate Inter-hospital Transfers Study seeks to accurately define and estimate the incidence and patient safety impact of potentially inappropriate inter-hospital transfers among medical patients who have undergone transfer to a large network of US hospitals, characterize and identify at-risk patients, and develop a toolkit of best practices to reduce potentially inappropriate inter-hospital transfers for future dissemination (e.g., by replacing it with safer alternatives).
Principal Investigator Stephanie Mueller and Co-Investigator Jeff Schnipper held an informal social happy hour to meet team members among the 18 sites participating in the study.
