HOMERuN at SHM Converge 2026 and Celebrations | | The Hospital Medicine Reengineering Network (HOMERuN) is a national network of Hospital Medicine investigators at 12 academic medical centers (AMCs) and 50 affiliated sites. | | |
HOMERuN convened meetings and social events among consortium members at SHM Converge 2026 to celebrate accomplishments and plan for future collaboration.
In this newsletter we will highlight:
- Events: ADEPT Study Meeting, Executive and Steering Committee Meeting, Pragmatic Trials Meeting, HOMERuN Happy Hour
- Awards and Celebrations
- Posters and Presentations: POINT Plenary, ADEPT, UPSIDE, HOMERuN Work Groups
| | At this year's SHM Converge Conference, the Achieving Diagnostic Excellence through Prevention and Teamwork (ADEPT) study team gathered to celebrate four years of incredible learning, growth, and success. The group recognized over 3500 reviews for diagnostic opportunities, the engagement of more than 150 hospitalists in ADEPT initiatives, and the collaboration on nearly 400 diagnostic cross-checks — with 1 in 4 participants reporting an impact on their patient's care. Over the next six months the study team will continue to codify this work at their institutions, and we will evaluate the impact of these programs on diagnostic performance at our sites. ADEPT site leads will remain leaders in diagnosis for years to come, and the investigators look forward to the additional exploration that comes from this stellar foundation. | | Executive Committee and Steering Committee Meeting | | The HOMERuN Steering and Executive Committees met during SHM Converge 2026 to discuss how HOMERuN can fill gaps in academic hospital medicine through focused initiatives like mentorship, research, and faculty development. Key next steps include engaging stakeholders, testing feasibility, and exploring comparator organizations over the coming months. We anticipate broader announcements around these strategies in August 2026 but welcome comments and thoughts in the meantime (via email to Tiffany.Lee@ucsf.edu). | | Representatives from several HOMERuN sites met to discuss ideas for pragmatic clinical trials. The discussion included the SPOT-ON trial, to compare spot check vs. continuous pulse oximetry among patients hospitalized on the wards (lead: Valerie Vaughn, Utah); and the COMFORT study on pain control (lead Akshar Rambachan, UCSF). Additional topics for consideration are POCUS for patients with dyspnea and volume-related admitting complaints, and comparative effectiveness of sleep medications among patients at risk for delirium. One application currently under review concerns benzodiazepine vs phenobarbital based regimens for ETOH withdrawal (lead: Valerie Vaughn). The group also discussed various funding agencies, including NIH, PCORI, DOD, industry, and foundations. | | To celebrate the collaboration among our team members, Alan Kubey and Andy Auerbach planned an informal social happy hour. | | HOMERuN Posters and Presentations | | Plenary Presentation: "Prevalence and Predictors of Potentially Inappropriate Inter-Hospital Transfers: Early Results from the POINT Study" featuring PI, Stephanie Mueller from Brigham and Women's Hospital. | | | | Poster: "Understanding Everyday Safety: An Ethnographic Study of Hospitalist Diagnostic Work" featuring Angela Keniston from University of Colorado. | | Poster: "Patterns in the Noise: Understanding Adaptive Approaches to Accurate Diagnosis in Hospital Medicine Using a Rapid Qualitative Approach" featuring Monisha Bhatia from UCSF. | | | | | Poster: "Local Implementation of a National Diagnostic Crosscheck Intervention to Increase Diagnostic Excellence" featuring Ellen Barbouche from University of Wisconsin and Andy Auerbach from UCSF. | | Oral Presentation: "Partnering for the Right Diagnosis: Engaging Patients to Improve Diagnostic Safety" featuring James Harrison from UCSF, Anuj Dalal from Brigham and Women's Hospital, and Katie Raffel from University of Colorado. | | Poster: "Informal Peer Diagnostic Second Opinion: Hospitalist Practices and Perspectives" featuring Taiju Miyagami from University of Colorado. | | | | | Poster: "Training Hospitalists to Conduct Peer-to-Peer Diagnostic Cross-checks: Early Multicenter Experience from the ADEPT Collaborative Group" featuring Andy Auerbach from UCSF, Jeff Schnipper from Brigham and Women's Hospital, Nathan Ziman from UCSF, Molly Kantor from UCSF, and Ray Klein from Maine Medical Center. | | Oral Presentation: "Diagnostic Cross-check: A Multi-Institutional Intervention to Facilitate Peer Second Opinion Discussions in Patients Experiencing Clinical Decompensation" featuring Allison Bond from UCSF. | | Poster: "The Long-Latent Lesion: Unraveling the Mystery of Marjolin Ulcers" featuring Ari Fish from Mount Sinai Hospital. | | | | | Poster: "Is Sepsis More Prone to Diagnostic Errors?
