Anticoagulation and Venous Thromboembolism Prevention 
Venous thromboembolism (VTE)—the formation of a blood clot in a deep vein—is the most common, preventable, hospital-related death. There is strong evidence that mechanical and pharmacologic prophylaxis can prevent the majority of VTEs. However, appropriate prophylaxis is ordered less than 41 percent of the time, and treatments often do not reach the patient.1

Anticoagulation medications are considered high risk due to their complex dosing, monitoring requirements, and inconsistent patient compliance. Science-based prevention and treatment strategies ensure the appropriate medication for the indication is selected, as well as the appropriate dose and frequency.2,3

Data show Adverse Drug Event (ADE) Anticoagulant rates have improved since the baseline period of 2019, while VTE Post-Operative rates have worsened since the baseline period of 2019 (lower rates are better).

Visit the HSAG HQIC Quality Improvement Innovation Portal (QIIP) to view your ADE Anticoagulant data by selecting the Measure Category of Adverse Drug Events and Measure of ADE Anticoagulant. Select the Measure Category of Other Harms and Measure of VTE Post-Operative (PSI-12) to view your VTE Post-Operative data.

Workplace Violence
Workplace violence is an increasing concern worldwide and poses a constant threat to healthcare workers, who are at higher risk due to dealing with overstressed people.4,5 Nurses are notably at risk because of their frontline position and persistent contact with patients and families.6 Approximately 57 nurses are assaulted daily in the United States—two nurses every hour.7

Unfortunately, workplace violence is underreported as a result of a widely held belief that it is a part of the profession.8 According to a 2021 American Federation of Labor and Congress of Industrial Organizations report on worker safety, rates of workplace violence in healthcare and social assistance settings have almost doubled since 2010.8
