Providers Must Apply for the Remaining Available Relief Funds - see link below
Our understanding is $40 Billion of the $50 Billion authorized for distribution has been distributed as of 5:00 p.m. on Friday April 24th.  $10 Billion remains available for distribution. Providers must submit an application via the portal to apply for additional remaining funds.
Application Portal for additional "General" disbursements:
The Provider Relief Fund Application Portal is to collect information from providers who received General Distribution payments prior to April 24th at 5:00 p.m. EST and who wish to be considered for an additional disbursement from the General Distribution.

Thus far, HHS has authorized the release of $50 Billion as a "General" Distribution:
$26 Billion on 4/10 (Most of you received a payment from this distribution);
$4 Billion on 4/17; and,
$20 Billion on 4/24.

If a provider did not receive any funds from the previous releases, the provider is likely ineligible for any of the remaining $10 Billion General Distribution. However, these providers MAY be eligible for future Targeted Distributions.
For providers who are able to enter the Application Portal, HHS is asking for the following information:
  1. A provider's "Gross Receipts or Sales" or "Program Service Revenue" as submitted on its federal income tax return;
  2. The provider's estimated revenue losses in March 2020 and April 2020 due to COVID;
  3. A copy of the provider's most recently filed federal income tax return;
  4. A listing of the TINs any of the provider's subsidiary organizations that have received relief funds but that DO NOT file separate tax returns.
Timing : Funds will NOT be disbursed on a first-come-first-served basis. HHS has specified that an applicant will be given equal consideration regardless of when they apply however, practices should consider submitting an application sooner rather than later.  HHS is processing applications in batches every Wednesday at 12:00 noon EST. HHS intends to distribute additional funds within 10 business days of submission for groups that are submitting revenue info through the application portal.
  Attestation Portal for funds already received (4/10, 4/17 and 4/24):

This portal is for all providers who received Provider Relief Funds as of 5:00 p.m. April 24th.  You will be asked to accept the
terms and conditions for receipt of these funds.  If the provider fails to complete the attestation, it will be assumed that the provider has agreed to the terms and conditions.

In Summary:
HHS was authorized to distribute $175 Billion from the CARES Provider Relief Fund to Providers and Suppliers enrolled in the Medicare or Medicaid programs.  These include hospitals, physicians, PAs, NPs, RHCs, FQHCs, other clinicians, facilities, etc.
