HHQI Resources: Top 5 Interactive Patient Tools

There are more than 1,000 high-quality educational materials in HHQI's complete online  Resource Library . So how do you wade through them all to quickly find the resources that are relevant to your agency's current needs? If you're looking for a specific resource, use the search bar at the top of the website. Otherwise, use the drop-down filters in the middle of the page to narrow your searches by topic, audience, language, etc. You can watch this short  6-minute tutorial for a demonstration. Please note, registration is required to access many of the resources, but it is easy and free!

One of the filters is Material Type. Click on the icon legend on the page for quick descriptions of the material types. Today, we will highlight the most downloaded "Interactive Tools" which are defined as "tools that require user input or include fill-in fields such as worksheets and forms." The top five include: 
  1. ZONE and Stoplight Tools (heart failure & diabetes) use green/yellow/red theme for recognizing and managing specific diseases.
  2. Patient workbooks & Home Telehealth: Disease Management Series (includes workbooks) provide self-management education on various diseases.
  3. Medication Schedules & Pocket Cards assist patients to manage medications with medication listing or to provide list to other providers.
  4. Action Plans are tools to assist patient activation into self-management and include a confidence and importance/efficacy ruler.
  5. My Emergency Care Plan is similar to the ZONE tools but uses symptoms vs. a specific disease which is appropriate for patients with multiple diseases.

June 2018 HHQI Agency of the Month

This month, HHQI recognizes Community VNA of Attleboro, Massachusetts for their dedication to improving home health care quality.

Home health agencies that are active in the HHQI National Campaign and closest to the  national, r isk-adjusted ACH 20th percentile and/or the national, risk-adjusted Oral Medication Improvement 80th percentile (or better) are eligible for random selection as HHQI Agency of the Month. 

Be in the spotlight.  Nominate your agency today , and if you are selected, your organization will be prominently featured here in HHQI Inside Edition -- which reaches nearly 13,000 subscribers -- and on the HHQI website. Your achievement will also be shared via HHQI's social media channels.
HHQI University Surpasses 560k CE Hrs Awarded

HHQI University students can enroll in nearly 50 different on-demand courses that provide education on evidence-based practices and integrating into day-to-day care and activities. There are courses for leadership, nurses, therapists, social workers, and aides. All courses provide nursing continuing education approved by Alabama State Nurses Association (ANCC provider). Several courses are approved for Physical Therapy ( ProCERT ) and Social Worker CEs (NASW).

The goal for HHQI University is to provide meaningful evidence-based education easily to providers to ultimately improve patient care. Today, more than 560,000 educational hours have been awarded since the inception of HHQI University in January 2015.

Upgrades Coming to HHQI Data Access

HHQI's secure Data Access system is getting an upgrade this summer. Here's what you should know now to plan for any needed adjustments in obtaining your reports:
  1. You will continue to see the monthly report for the current year (2018), but the historical OASIS-based reports (ACH, OM, FLU & PNE Immunization Reports, Cardiac Risk Reports) dating from 2010-2017 will be condensed to annual reports showing all 12 months' data for that specific year. 
  2. The Data Access System will be unavailable from June 18 through June 20. This will impact access to the support resources, tutorial webinars, FAQs, OASIS-based reports, and the HHCDR.

Statin Therapy for the Prevention & Treatment of CVD

Advances in cardiovascular treatment options occur each day, but few interventions have stood the test of time like statin therapy. During our next CardioLAN webinar, you'll get a refresher on cardiovascular disease basics and then learn about newer findings related to statin therapy, such as which of your patient populations could benefit most and its effects beyond cholesterol management.

You'll also learn how to best integrate cardiovascular evidence-based practices into your agency and how to help your patients understand all of the benefits of statin therapy. Our featured experts -- Chelsea Leonard and Matthew Holland -- will also discuss non-pharmacologic options and address your questions during this 60-minute presentation.

EARN FREE CONTINUING EDUCATION CREDITS

This activity has been submitted to Alabama State Nurses Association for approval to award contact hours. Alabama State Nurses Association is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.

To receive CE credit, participants must watch the webinar and enroll in the corresponding course in HHQI University's Cardiovascular Health course catalog (available after the live webinar broadcast) to complete related coursework. Step-by-step instructions will be reviewed at the end of the webinar.

In anticipation of the July 2018 Release, the Common Working File (CWF) Hosts will not process claims beginning Friday, June 29, 2018 through Sunday, July 1, 2018. During this period, which is commonly referred to as 'dark days,' the CWF Hosts will install the July 2018 Release, complete weekly/monthly/quarterly processing activities, and perform scheduled data center maintenance.
