As we prepare for the beginning of the 2021-2022 school year, below you will find some important information and reminders from the Saint Andrew's Health Office:
Health Forms

Please click the link below to view the required health forms for the upcoming school year. Parents are responsible for completing ALL of the health forms included in the tabs under the Grade Level corresponding to your student(s). Please note that Parents are also responsible for arranging any necessary forms to be completed by your Pediatrician, (i.e. Health Assessment Part II, Immunization Record, Medication Administration Orders, Asthma/Allergy Action Plans). *For New Students, please complete ALL of the forms included in the tabs under the New Students section. 

* In addition to the Maryland Department of Health required health forms, we have included a new Saint Andrew's Health Room Information Form to be completed by Parents for each of their students. This form will be used by the School Nurses to ensure complete health and wellness considerations for every student as well as your preferred Health Room Visit contact information. 
Covid-19 Vaccinations

We encourage all eligible persons to be fully vaccinated against COVID-19 for the upcoming school year. If your student(s) are fully vaccinated, we ask that you send the Health Office a copy of the vaccination card. If your student(s) are in the process of becoming fully vaccinated, we ask that you inform the Health Office of your student(s) status and follow up by sending a copy of the vaccination card when available. This information is especially important in helping the School Nurses manage any possible cases or potential cohort closures that may arise. We are asking for a copy of the vaccination card as a way to help the School Nurses keep an accurate record of vaccinated students per cohort. If you have any questions or concerns pertaining to COVID-19 Vaccinations for students, please contact the School Nurses in the Health Office.

Health Monitoring

We will work in close collaboration with the Anne Arundel County Department of Health to address any suspected or confirmed cases of COVID-19 at school. A key component is the commitment of our families to informing the School Nurses in the Health Office when a child experiences COVID-19-like symptoms or has had a known exposure to COVID-19. We will not be taking temperatures during our morning car line as was our protocol last year. We are asking all families to monitor your student(s) for any illness or unwell symptoms prior to sending your student(s) to school. * In keeping with our health monitoring protocol from last year, Parents will need to contact the Health Office with any illness symptoms, absences, isolation/quarantine orders, or other concerns that could potentially effect the health and safety of all students, faculty, and families of the St. Andrew's community. Please direct any health concerns or absences of students to the Health Office first and foremost, in addition to Teachers and/or the Main Office accordingly, so that we may address them appropriately and in a timely manner.
