DeeDee Velasquez-Peralta is the Patient Services Manager and has been with the Network since October 2010. DeeDee has a Masters Degree in Social Work and has more than 20 years of experience in healthcare settings, including dialysis, acute care, rehabilitation and disability advocacy. DeeDee leads the Patient Services Department providing assistance with patient grievances and facility concerns.

She collaborates on and supports the Network's Quality Improvement Activities, providing resources to support patient engagement and educational resources, like the Heartland Headlines patient newsletter and My Life, My Choices: Knowledge is Power -- My Kidney Kit and My Kidney Calendar for patients. Recently she has taken on additional community outreach responsibilities and leads of the Patient Advisory Council and Network Patient Representative Program and facilitating patient engagement at both the Network and the facility level.

Improving Outcomes through
Patient Engagement

When patients are active in their own health care they have better outcomes. This year, the quality improvement activities have included a patient partnership component and the selection of Centers for Medicare and Medicaid Services (CMS) driven patient engagement objectives to incorporate into facility improvement activities. In order to support this work, the Network has been presenting on and developing resources with the CMS objectives in mind.

Speak Up and Speak Out: Patient Engagement in Infection Prevention

Infections are the second leading cause of death in hemodialysis patients. It’s important for healthcare workers to understand and follow infection control practices, but what about the person at the center of it all? 

Have you educated your patients on proper infection control practices, so they know when to SPEAK UP ? Does the culture of your facility encourage patients to SPEAK OUT on their own behalf? 

This video from the Agency for Healthcare Research and Quality will guide you and your team in creating a Speak Up and Speak Out culture in your facility. 

The Conversation Starter from the Making Dialysis Safer for Patients Coalition is a great resource to get you started.
Network Patient Representative Program (NPR) Connection Calls

This month’s NPR Connection Calls will be Sept. 13 and 14 at 1pm CT. The goal of these calls is to provide an opportunity for NPRs to connect with the Network and each other, share ideas, and learn what they can do at their clinic to support other patients.

In addition to sharing information on the Network QIAs, our focus in September will be the on “Patient Rights and Responsibilities” to share ideas to plan an activity in connection with the October topic of the month. Please encourage your NPR to attend one of the calls and staff is always welcome to join, too.

My Kidney Calendar | September
Each month the Network shares ideas based upon the "topic of the month" in conjunction with the 2018 My Kidney Calendar.

September's monthly topic of the My Kidney Calendar was "My Plan: Be Prepared". Highlighted resources and interventions included information on emergency preparedness resources and activities to engage your patients.

The Network recognizes that facilities are always working hard to develop and implement creative and engaging activities for their patients, through bulletin boards, games and lobby days to name a few.

We encourage you to review the monthly "Topic of the Month"   sheet with your Network Patient Representative and brainstorm ideas on how to best address the topic in your facility. 
Patient Subject Matter Experts,  Providing the Patient Voice
The Networks Quality Improvement Activities (QIA’s) require a patient perspective aspect. The patients that support the work of the Network are patient subject matter experts (PSMEs) and they provide valuable feedback for our projects.

One such PSME, is Mike Bronson.

Mike received a living donor transplant in October of 2009 but prior to transplant was on home dialysis. His journey with home dialysis began in July 2009 where he did CCPD (continuous cycler peritoneal dialysis) but later changed to CAPD (continuous ambulatory PD).
 
Because of his perspective on home peritoneal dialysis, the Network asked him to evaluate a home resource in development. The purpose of the My Life, My Choice: Is Home Dialysis Right for Me? poster is to assist potential home dialysis patients to determine if home dialysis is right for them. 

Mike shared his perspective with us regarding the layout, font selection and readability of the poster. He also shared that being very simple is good for patients that might be uremic or overwhelmed with information. He liked the QR code and liked that it tells them to go to www.mykidneykit.org for more information.

End Stage Renal Disease National Coordinating Center Quality Improvement Activity Learning and Action Networks (ESRD NCC QIA LAN)

The ESRD NCC hosts a series of calls based on topics of the CMS Quality Improvement Activities (QIAs). These include: Blood Stream Infection (BSI), Home Modality, Transplant, and Depression. ALL ESRD facilities will receive invitations to these meetings because improving the lives of dialysis patients in these areas is a global goal. The expectation is that project facilities join these calls but the calls are optional for non-participating facilities. Please also share these calls with your patients.
