Meet Charles, an 87-year-old heart patient – and avid fisherman

Charles recently underwent coronary artery bypass surgery. Right before his discharge, a Care Transitions Intervention Coach met with him to discuss his goals and found that his biggest motivator was getting back to fishing with his long-time buddies. 

Knowing that isolation can seriously impact older adults’ wellbeing, the CTI Coach prompted Charles to think of ways to build up his strength so he could soon wade back into the stream with his friends.

 

On subsequent visits with Charles, the Coach also discovered that he avoided taking his blood thinner on fishing days because he often cut himself on creekside brambles. The coach skillfully guided Charles to script questions for his physician, and Charles got the idea to pick up a heavier jacket to protect his skin from thorns. 

 

The Coach documented these activities in the CTI+ technology, tracking their progress together. The CTI+ metrics help quantify the impact of Charles’ new-found skills on managing his health –  and demonstrates to payers the overall value of the coaching.
