Ensures a positive first impression for every patient that enters the clinic. Duties include, but are not limited to, welcoming and registering patients, accepting payments and effectively using the electronic health records system. This position requires exceptional critical thinking, communication, patient relations, organizational, and time management skills.

Assists all patients in need of Behavioral Health (BH) Services by acting as a patient and program advocate and navigator. Assists in developing BH work flows and supports with scheduling. Addresses barriers to BH care by providing advocacy and coordination of appointments and referrals, prepares charts for patient visits, assists the BH providers with workflows, and serves as a strong and flexible member of a health care team. The IBHCC demonstrates exceptional critical thinking, clinical, patient and provider relations, organizational, and time management skills.

Maintains and expands current enrollment activities and enrolls eligible patients and service area residents into affordable health insurance coverage under Medi-Cal and the Affordable Care Act (ACA) as well as other programs such as CalFresh, formerly known as food stamps. The Outreach and Enrollment Team provides integral coordination for the Programs Departments various programs and initiatives. This team leads Santa Cruz Community Health’s Food Distribution program.

Responsible for the overall management of the Patient Services Team, which includes the Call Center, the Medical Records staff, and the Insurance Verification Specialist. Creates a work environment that supports excellent internal and external customer service. The PSM serves as a resource for all Patient Services staff and addresses administrative issues and concerns as well as applicable patient grievances.

Manages an assigned panel of patients who meet criteria for Case Management services. Utilizes screening tools and evidence-based practices to support patient-centered care and goal development. Designs and implements care plans that improve the patient experience, improve health outcomes, and reduce barriers to care. Provides risk assessment and crisis intervention services as needed. Consults and collaborates with members of the patient’s care team, including developing shared treatment plans, goals, and interventions. Consults and coordinates with community systems to facilitate linkage, manage referrals and advocate for patient needs, with a focus on supporting identified treatment goals.

Weekly Mailing
The Romero Institute is looking for volunteers to assist with weekly mailings, preferably 2-3 hours a week. Volunteers can assist at a time that is convenient for them during regular business hours from Monday - Friday between 9am - 5pm. Click here for more information and to apply.
