During your appointment with a Heritage Valley Multispecialty Group (HVMG) Primary Care physician, you will be provided with a Health Needs Assessment to complete and discuss with your provider. The goal of a Health Needs Assessment is to improve health and help ensure that patients receive the health services that they may need.
If a need is identified, your provider will refer you to our Care Management Team, who will contact you by phone to determine how to best help you and provide the tools and resources you need to overcome any barriers and put your health first! Factors that can affect your health include your environment, lack of utilities, lack of shelter, poverty, lack of transportation or limited access to food.
