With appointment-free, walk-in convenience and locations close to where you live and work, Heritage Valley ConvenientCare Walk-In Clinics, located within our Medical Neighborhoods, can accommodate the healthcare needs of today’s busy families in a cost-effective manner.

Designed to supplement healthcare access for our patients 18 months of age and older, our ConvenientCare Clinics are specially designed to address less urgent healthcare needs at your convenience. While minor illnesses and injuries are typically the most common reasons for patients to seek care at our ConvenientCare Clinics, we also provide certain vaccines, physicals and diagnostic testing.

During your appointment with a Heritage Valley Multispecialty Group (HVMG) Primary Care physician, you will be provided with a Health Needs Assessment to complete and discuss with your provider. The goal of a Health Needs Assessment is to improve health and help ensure that patients receive the health services that they may need.  

If a need is identified, your provider will refer you to our Care Management Team, who will contact you by phone to determine how to best help you and provide the tools and resources you need to overcome any barriers and put your health first! Factors that can affect your health include your environment, lack of utilities, lack of shelter, poverty, lack of transportation or limited access to food.

Heritage Valley Pediatrics provides high quality pediatric care at four convenient locations. Sick visits for established patients ages 6 months and up may be easily self-scheduled through the Heritage Valley app. The first step is to download the iHVHS (iPhone) or aHVHS (android) app on your phone and create a “My Care Link” account. Once your account is active, you can add your child to your account and schedule an appointment with Heritage Valley Pediatrics.
