2026-2027 MP Student Directory Submission Form

Dear Shelter Rock Families,

Welcome to the 2026/2027 school year! Each year, the Shelter Rock SCA publishes a Student Directory to help our community stay connected.

To be included in this year's directory, please complete the Shelter Rock SCA Student Directory Form by October 1. We cannot guarantee inclusion for any submissions received after this deadline.

Important details for submission:

One form per family: You can list multiple students on a single form, so you only need to submit it once.
Complete information required: All required fields—including Student Name, Address, Phone Number, Parent/Guardian Name, and Email—must be completed to be included. Entries with incomplete information will not be included.
Privacy: The directory is strictly for school-related communication among families and may not be used for commercial or public distribution.

Dear Secondary School Families,

We are putting together the 2026–2027 Secondary School Student Directory.

If you would like your family's information included in the directory, please complete the form by Thursday, October 1.

Secondary School Directory Form

Dear Shelter Rock 6th Grade Parents/Guardians,

6th graders will have their photo taken on Thursday, October 15th. Suggested attire for girls is a white blouse or button down shirt, with dark pants or skirt, and for boys, a white button down shirt, dark tie, and dark pants. Please note that this will be their yearbook photo.

In addition to individual and class pictures, on Wednesday, October 14th, the 6th graders will receive their personalized sweatshirts and they will take a picture as a class with all other students in the 6th grade.

There are no forms needed to order a photo package of your son or daughter. A few weeks after photo day, you will receive a notification by email that your proofs are available for viewing. If you are interested in ordering a photo package, you can do so

online. Since this will be used as their yearbook photo, please be aware that you must choose the blue background when ordering pictures.

If your child is absent or would like a retake, retake day is on Wednesday, November 18th, 2026.
