Eligible students have three options:

• Opt out of the plan if you already have comparable health & dental coverage

• Opt back in if you opted out in previous years

• Add dependents to your plan (spouse and/or children)

Forms must be submitted online before the deadline on September 30, 2022 at 4:00PM.
