Pregnant and parenting teenagers age 19 and younger

First-time, low-income pregnant people, before week 28 of pregnancy

Low-income pregnant or parenting people with children up to age 5 who have social and/or medical risk factors.

We Are Serving Our Clients In Person!

We are happy to announce that our three maternal-child home visiting programs resumed in-person client visits effective July 1, 2021. After many months of virtual and phone visits, and extended absences due to Covid-19 response activities, our home visitors are back in the field and providing in-person support, coaching, and assessments.

All of our Home Visiting Programs aim to increase immunization and breastfeeding rates for infants, improve health outcomes for mother and baby, and increase child developmental screenings and referrals, among other metrics that vary by program.

Kembly Mahiri is the Program Supervisor for Teen Parent Connection. She has been working for TPC as a home visitor and supervisor for 23 years. Her passion are children and teens. Kembly grew up in Sonoma County as a child of migrant farmworkers and deeply loves her community.

AManda Elderkin is the Program Supervisor for Nurse-Family Partnership. AManda, RN, PHN, BSN, has worked with new families in their homes and in birth centers since 2004 and was a Nurse Home Visitor for Nurse-Family Partnership before becoming supervisor in 2020.

Laura Turner is the Program Supervisor for Trauma-Informed Approach Public Health Nursing (TIA PHN). Laura, MSN, RN, PHN previously worked in public health research, and transitioned into nursing in 2013.
