Your organization is invited to participate in any or all of the following ways:

Submit names of those who passed away this year who were currently experiencing or had a history of homelessness. Each name will be read and displayed as part of the program.

  • Names can be in any format, including first name only, initials, or nicknames.
  • Please include any additional information that you would like included in the name display, (such as a photo, DOB/ DOD, veteran status or occupation, a sentence about them, etc.)
  • Please submit names by Tuesday, December 9th.
  • Submit names to policy@cfhnyc.org.

Submit a eulogy for an individual lost this year. Eulogies are an opportunity to honor those we’ve lost and are a powerful part of the program.

  • Eulogies should be brief, 1 – 2 minutes in length.
  • If submitting a eulogy, please identify someone from your organization, either a staff member, senior leadership, or program participant, who is available to read the eulogy during the program on December 18th.
  • Examples and support for writing a eulogy can be provided, if desired.
  • Please share if you plan to submit a eulogy by December 9th so there is appropriate time allotted in the program.

Invite clients and staff to submit a poem or short story that pays tribute to those we’ve lost or focuses on the issue of homelessness.

  • Three submissions will be selected, and the author will be invited to read it as part of the memorial’s program, to be held on December 18th.
  • Submissions should be sent to policy@cfhnyc.org.
  • A flyer is attached that can be shared at your programs.

Invite clients and staff to submit visual art to display with the Wall of Names.

  • This year, a new component will be added: the collection and display of artwork that adds a personal touch to the memorial.
  • Artwork will be included on the name wall (i.e. the wall where names of individuals that are being honored at the memorial).
  • Artwork that can be attached to a wall, such as drawings, paintings, pictures, poems, songs, etc. You can either donate the artwork to the memorial, and they will be used for future observances OR you can donate the artwork for this memorial only and an arrangement can be made for it to be returned. If you are donating the artwork for future observances, please do not include a date on the artwork as each year the day of HPMD changes.
  • Art will add to the beautification of the memorial and are looking for donations from now through December 5th.
  • Please reach out to Kandra Clark, kclark@urbanpathways.org or text at 646-263-9732, directly to discuss ideas, ask questions, provide feedback, or discuss options for dropping your art off or coordinating a pickup.
