PDF of Uniform Application
HCPF has been alerted that the PDF version of the Uniform Application posted last week contained references to CICP. These have been removed, and an updated version has been posted on the Hospital Discounted Care website. HCPF apologizes for any inconvenience this may have caused.
As a reminder, to continue meeting statutory requirements, all hospitals will need to update the applications posted on their website using either Version 3 of the Excel Uniform Application or the updated PDF version. Additionally, the updated Patient Rights documents need to be posted in both English and Spanish. HCPF has provided Google Doc versions of the Patient Rights that hospitals can download and more easily enter their information before transferring to their own letterhead and/or posting. Hospitals that are unable to update the Patient Rights contained in their billing statements by today need to send the date by which the billing statements will be updated to hcpf_HospDiscountCare@state.co.us. Having this date recorded will be important for auditing purposes and will also ensure that hospitals remain in compliance.
