REMINDER: Annual Reporting Data Due November 1, 2025 for 6/30 FYE Hospitals

Greetings,

The Department of Health Care Policy and Financing (HCPF) would like to remind hospitals with fiscal year ends (FYE) of June 30 of the upcoming Annual Reporting Template deadline for FY 2025 data. Hospitals must submit the annual report template and audited financial statements by November 1, 2025. The Medicare Cost Report is due by December 29, 2025.

The Annual Report template for the 2025 Annual report template is now available on the Myers & Stauffer’s Hospital Information System (HIS) Portal. You can also access it through the HCPF Hospital Financial Transparency webpage under Reporting Tools. All submissions must be completed through the HIS Portal. For assistance with portal access or submissions, please contact the HFT Help Desk.

Updates to the 2025 template include:

1. Utilizing the list provided below, hospitals are to include them in the template twenty-five major service lines and are required to report their status. If a hospital has another major service line that is not included in this list you may add it within the template to  replace a service your hospital does not provide from the provided service line list. 
2. More instructions on new tabs and also pop-outs for each column to help hospitals as they submit information.

Due Dates to HCPF

Additional information on the Annual Reporting and Quarterly Reporting Templates along with other documents related to hospital financial transparency requirements can be found on the HCPF Hospital Financial Transparency webpage.

FY 2025 Data Collection

Hospitals must submit the following for the upcoming data collection: 

1. Completed Hospital Financial Transparency Annual Template (120 days FYE). 
2. Audited Financial Statements for FY 2025 (121 days after FYE).
3. Medicare Cost Report for FY 202 (180 after FYE).
