The Human Factors Analysis and Classification System (HFACS) is recognized as the global standard for conducting a thorough and credible human factors analysis of any adverse event.

The HFACS Framework has direct application in accident investigation and analysis, risk reduction, quality and productivity improvement. HFACS Workshops offer training on the innovative HFACS framework and methodologies. The intensive, two-day workshops teach updated cutting-edge techniques to help organizations identify the causes of errors and develop preventative measures to lower risk and improve performance. Registration is available for online and in-person HFACS Workshops.
Attendees will learn how to: 
  • Integrate human factors and system safety concepts into the root cause analysis (RCA) process
  • Utilize the Human Factors Analysis & Classification System (HFACS®) to identify systemic causes of human error during accidents, incidents, and/or near misses
  • Integrate HFACS into traditional RCA tools like the fishbone diagram, fault trees, and link analysis using HFACS
  • Develop a human error database and tracking system for monitoring and evaluating performance improvement efforts
