Our Annual Wellness Screenings are right around the corner and it’s time to schedule your appointment.

If you have medical coverage provided by DART you are required to participate in a Wellness Screening/physical

This can be completed 1 of 2 ways:

OPTION 1: COMPLETE AN ON-SITE WELLNESS EXAM

The first way is to participate in one of the Wellness Screenings provided by DART.

The dates and times for these screening are listed below and sent on a card mailed to your home address earlier this week.

OPTION 2: HAVE YOUR PERSONAL DOCTOR COMPLETE AN EXAM & FORM

The second way is to complete an annual physical with your physician.

If you would like to visit your own physician, you will need to take a Physician’s Form with you for them to complete and return to Unity Point.

If you have already completed your annual physical with your physician between January 1, 2023 and now, that information can be used. You will need to supply your physician with the Physician Form, so they can complete and return it to Unity Point.

All Physician Forms must be submitted to Unity Point by November 1, 2023.
