Humana Network Relationship Termination

Dear Patient,

We regret to inform you that, despite our best efforts, we have been unable to reach an agreement with Humana regarding contract terms that would allow us to continue delivering the high-quality health care you expect from your trusted physician at South Florida ENT Associates.

If you are enrolled in a Medicare Advantage Plan and would like to continue treatment with your South Florida ENT provider, you may select a new Medicare Advantage Plan by March 31^st, 2024. To assist you in this transition, please find a list of Medicare Advantage insurance plans that we accept:

If you would like to express your frustration, please call Humana's customer service number located on the back of your insurance card.
