Ontario’s Chief Coroner releases the first annual report of the Construction Death Review

This milestone report reviews the tragic deaths of 26 workers who fell from a height between 2017 and 2023. It provides important insights and actionable recommendations aimed at preventing future fatalities in Ontario’s construction sector. The report was released by the Office of the Chief Coroner on July 18.

The Construction Death Review represents a shift from the previous mandatory inquest process to a collaborative, timely, and system-focused review. For the first time, government, labour, employer, and safety partners have come together in a structured process to examine these tragedies and issue recommendations grounded in practical experience.
