IMPACT Connect is Southeastern Wisconsin's Community Information Exchange - a collaborative partnership of agencies combining efforts to share information and coordinate care so people have better access to the vital resources they need, resulting in improved health and well-being.
Community Information Exchanges use a shared governance model which allows for joint decision-making that benefits the entire partner network. IMPACT Connect is governed by a Board of Managers with engagement from Standing Committees.
Board of Managers provides oversight, as well as creates committees and work groups to accomplish set objectives. The Board is comprised of representatives from each foundational partner, as well as representatives from key public and community organizations:
- Froedtert
- Advocate Aurora Health
- Children’s Wisconsin
- Sixteenth Street Community Health Centers
- Milwaukee Healthcare Partnership
- United Way of Greater Milwaukee & Waukesha County
- Representation from each standing committee
Standing Committees provide an opportunity for member organizations and community stakeholders to be involved in data sharing, process implementation and policy review. Committees may include representatives from foundational partners, member agencies and the community.
Outcomes Committee
The Outcomes Committee gathers objective and subjective data, and then interprets that data in the context of the communities served by IMPACT Connect. The group makes data-driven recommendations to the Board of Managers.
Integration Committee
The Integration Committee builds collaboration among member agencies. Through these coalitions, ways in which to enhance program navigation and system improvement are explored. The next meeting scheduled for Wednesday, June 29 from 10 to11am will focus on using network data to inform evidence-based strategies that promote the best use of multiple care systems.
