THINGS to KNOW and DO
Thank you to everyone who joined us for the PQCNC Kickoff meeting of the Cardiac Conditions in Obstetric Care Initiative! It was great to have so many interested people in one room.
For anyone who was unable to attend, please review the slides from the afternoon sessions here and download the Kickoff Packet. The documents in the Kickoff Packet are also available on the Team Resources Page with additional background resources here.
Then, meet with your team to draft your initial Quality Improvement Plan to turn your thoughts and ideas into actionable steps to take in the next few weeks as you get your project moving.
Whether or not your team was able to attend, please complete the following by February 21:
1 Email a picture or copy of your initial Quality Improvement Plan to qiadvisor@pqcnc.org
2 Submit your Team Roster if you haven't yet
3 Submit your initial Snapshot Survey if you haven't yet.
4 If you have not yet please let us know which two team members should have access to Life QI (your Data Entry Contact on the Roster and optionally one other team member) - email those names as soon as possible to qiadvisor@pqcnc.org so that we can create accounts for you prior to training.
5 Register your data entry person and other interested team members to attend the virtual Life QI Data Training Session, February 12, 11 am - 12:30 pm
6 Prior to the training, review the preliminary training video, Cardiac Conditions in Obstetric Care Data Collection Tool, and Cardiac Conditions in Obstetric Care Data Dictionary
7 Attend the virtual Life QI Data Training Session, February 12, 11 am - 12:30 pm that you registered for above
8 Complete your first Monthly Leadership Report, a template for your team to highlight and share what you focused on each month. Reports are due by the 15th of the following month (for example, January 2025 report due by February 15, 2025)
