Dear St.Therese PK-7 parents,
As part of our preparations for the 2020-2021 school year, we are improving the way student health records are submitted. St.Therese has been working this school year with Magnus Health to make it possible for parents to submit their child(ren)'s health records online.
The Magnus Health portal is available for you to submit your child(ren)'s health records now. We are asking parents to setup their accounts, complete all required information and upload any forms by
August 1, 2020
.
We highly recommend you begin this very important task now while you have time.
Some health forms might require a visit for form or information from a health care provider, which takes time. So, it would be a good idea to find out what is needed so you have enough time to gather/submit information. Read on to find out what you'll need to get started.
Simple instructions are provided to create a username/password for the Magnus mobile app plus a link to view a short video on how to use the mobile app. Once you download and set up the mobile app, you could use the camera of your mobile phone to take picture of medical records or forms that are required and submit them easily.
Once you get into the Magnus Health portal, you’ll find quite a bit of your child(ren)'s information are already in your account. Our staff uploaded this year's health information on the portal. You’ll need to add some additional personal and medical information. Depending on your child's grade/age, participation in extracurricular activities, etc., you also might be required to submit additional medical forms. The good news is that once all your information has been submitted, you'll only need to update information moving forward.

We appreciate your cooperation and encourage you to get started as soon as possible as you might need to retrieve documents, complete forms, get information/forms from your child's health care providers, etc.
All required information and forms must be submitted on the Magnus Health portal by August 1, 2020
