In an effort to increase awareness of our providers/specialties available to patients, please download the provider directory for your front desk areas. Note that this directory is an interim solution ahead of the BRAND NEW Tri-State Website coming in July. If you have changes, please reach out to your office manager with the updates. Monthly updates will be sent May and June. Thereafter, the website will be your digital directory with the most updated information and the ability to search/print results by providers/specialties/locations when referring patients.

 Hard copy printing tips:
  • Download the directory document and open in Excel.
  • Print each workbook individually. 
  • Use both sides printing, landscape orientation, fit all columns on one page.
  • In page set up under “sheet” check off gridlines.
