Connie’s Clinical Information and InContext App have several new features!

Clinical Data Settings
Organizations can now hide data sent from their own organization in their InContext App or, if viewing within the Connie Portal, in the Clinical Information App.
This feature is applicable to the Encounters and Health Records tabs under the Clinical Data section. 
Social Needs Data Section
A “Conditions” subtab under the new Social Needs Data section displays Social Determinants of Health information using ICD-10 z-Codes from ADTs. When used, these codes will highlight whether the patient has experienced problems related to education, literacy, employment, housing, psychosocial circumstances, and other social issues that have a known impact on health.

eReferral Display
eReferrals made through Connie will be displayed under Care Coordination.
No matter who initiated or received the referral, the patient’s care team from any facility will be able to see if their patient has been referred to homecare services and the status of the referral. Even though the eReferral process is conducted only in the Connie Portal, portal and InContext users have access to Referral History as read only.

Table of Contents
Continuity of Care Documents (CCDs) displayed in the Structured Documents subtab now include a Table of Contents to improve navigation to important sections of the CCD. Click on a section header in the table of contents and you will jump to that section of the document.
