If you plan to pick up your student early or want your student driver to leave campus early, you must send in a note the day early release is requested. A fill-in-the-blank form is provided below. If you do not want to use this form, that is fine, you may send a handwritten note.


  • No students will be dismissed after 3:00 pm on Monday, Tuesday, Thursday, and Friday. No students will be dismissed after 2:00 pm on early release Wednesday.

  • Your student must turn in their note to the front office before 9:15 am and have their planner stamped. A stamped planner is their pass out of class. Complying with this process will ensure your student is in the lobby waiting for you to pick them up.

  • Driving off Campus - A parent note is required when a student needs to drive off campus during school hours. Early release notes must be turned in by 9:15 am. Parents will always be contacted before the student receives a pass to drive off campus. Parents, please be available and have a current phone number listed on the note.

  • In an effort to keep the classroom a focused and productive environment, please do not ask the front office to interrupt class to ask the teacher to send your student for checkout.

***  Volunteers are not the only adults needing school access  . ***

Approved School Access ensures the safety of our students, faculty/staff, and our campus. Effective 2015,  all visitors and volunteers must have school access. Approved school access allows you to enter the school campus beyond the front lobby.

 Do you ever???
  • Attend a meeting with Administration or Guidance
  • Attend a PTSO meeting
  • Volunteer time to a Club or Booster on campus
  •  Proctor in the classroom, help in the bookroom, library, etc.
  •  Drive Team members to a game or tournament
  • Volunteer for Special Events (Ticket Sales, Concessions, etc.)
  • Help at a School Sponsored Event (Performance, Camp, Running of the Knights)

If you answered “YES” to any of the above scenarios, it is important to apply for school access online BEFORE your meeting/activity/volunteering. Please go to the school district’s website:

Please note:  You are unable enter beyond the front lobby until your form is cleared in the system. Please allow 2-3 weeks. Once cleared, a notification email will be sent to the email address provided on the application. All approvals are valid for three years.

The Creekside Guidance Department has recently launched a Schoology page. Parents and students are encouraged to join the Schoology page to receive the latest updates.

We encourage you to check our senior website frequently for updated information regarding important dates and information. 

Community Service Hours should be turned in frequently or as earned
to Ms. Hemmer in Guidance. We want to prevent a bottleneck in April.

This competition is for seniors only. Please note the following expectations:

  • All of students will need to email Mr. Savage a draft of their essay by 11/1/19 to be considered for the contest.
  • Contestants will also need to be prepared to deliver their speech to a few CHS teachers before school on 11/5/19.
A representative will be on campus November 8th during lunches to fit and take varsity jacket orders.

Junior Class Rings:
Dear Parents/Guardians, Jostens is our official class ring company. Please note, Jostens will be on campus for ring orders during all 3 lunches 11/14 (when final payment is due).

All rings require a $60 deposit and the balance must be paid before delivery. Jostens has free insurance included with each purchase and a lifetime warranty. You can go ahead and design your ring at and bring the printed order form along with your deposit to order day.

SAVE the DATE: Whoever purchased a class ring, the ring ceremony will be on 12/10 at 8:30am. If the student cannot attend the ceremony, they may pick up their ring from Jostens on 12/13 during lunches.

Boys Basketball tryouts will be held Monday, November 5 th at 5:30 PM. All athletes must be cleared on Athletic Clearance prior to tryouts. See the athletics website under forms for instructions.

Wrestling Tryouts will be held Monday Nov 11 th from 3-5:30pm and Tuesday, Nov 12 th from 4 – 6:30pm. Must have all requirements competed and cleared on athletic clearance before Nov 8 th and must indicate wrestle as a sport.

AP Capstone informational meeting
for prospective students and their parents (current 10 th graders only)

Our annual Barnes & Noble Bookfair will be on Saturday, December 14 at the San Jose location. Please support our media center by mentioning Creekside at the register.

Interested in taking Dual Enrollment classes for the 2020-2021 School year…..
  • Creekside High School partners with St. Johns River State College for students to take DE classes either on Creekside’s campus or on SJRSC campus (online or physically at the college campus)
  • Students can participate in DE for 3 academic school years or 60 college credit hours – whichever comes first:

  • Students must meet all eligibility prior to being able to participate in DE classes
  • DE classes replace the high school level class but offers students both a college credit and high school equivalent credit for each DE class
  • AS a dual enrollment student, you will begin to earn college credit and also begin your college GPA
  • As a dual enrollment student, you are considered a college student and all classes/grades are governed by the college (meaning grades are not on HAC). This means you must abide to the colleges grading policy, attendance policy testing policy and college timelines.

Annual Food Drive, November 16th
Volunteers needed to: place grocery bags and explanatory notes on mailboxes in designated neighborhoods on the 13th & 14th, collect food donations on the morning of the 16th, inventory and box the food donations at our two collection sites for pick up by 3 local Food Pantries. 
Volunteer to help others in St. Johns County so they may have needed food for their families. To volunteer, use the registration link under Food Drive on the Home Page.
4th Annual Turkey Trot Race, Thanksgiving Morning in Nocatee
Volunteers needed to help: set-up and clean-up, hand out water to the racers, direct racers along the course, wear the turkey costume while greeting bystanders and encouraging racers. 
Come join this family-friendly event and earn service hour certificates.

Annual Giving Tree Initiative
Volunteers need to help: distribute paper ornaments with wishes from non-profit organizations serving Elderly, Veterans, Homeless, Abused, Children and Animals in St. Johns County to donation host sites in mid-November. During the 2nd week of December, volunteers needed to: collect donations from the host sites, deliver to sorting location, inventory, sort and box donations for the individual non-profit organizations.
  • This is a great way to share the joy of the holidays with others.
  • Service hour certificates will be available. 
  • Volunteer registration will open at the end of October at the link under Giving Tree on the Home Page.
