Patriot Act Information Form

Attention Lenders:


This Bulletin references requirements regarding the Patriot Act Information Form.


The form is required to assist the government in fighting the funding of terrorism and money laundering activities. Federal law requires all financial institutions to obtain, verify, and record information that identifies every borrower.

 

The Patriot Act Information Form must be completed, signed and provided prior to NJHMFA purchase of a loan. The individual signing the form must be the person verifying the borrower’s identity (i.e., loan officer, closer, etc.). The signature must be on the last page of the form, not the instruction page. A notary signature and stamp is not required.
