Dear Elementary School Parents,
Spring is here and the 2021/22 school year is almost over. It's hard to believe I have been at LNC Elementary School for over 5 months serving as the nurse for your student.
To save you time and effort, we are asking you to complete this form giving LNC permission to use/administer over-the-counter medications, ointments, wound care, eye wash, etc. for your student. These items are stocked and available in the ES Health room and on field trips. The form gives us permission to use/administer OTC medications as needed and will be in effect the entire time your student is at LNC. The medications listed are common medications and the dosage given will be based on your student's weight. This will eliminate the need for you to fill these papers out at the beginning of each school year and before every field trip. This form will not replace the required paperwork for prescription medications such as epinephrine injectors, inhalers etc., which require a prescription and a physician's authorization.
