, we are prioritizing the health and well-being of our staff coming in to the office, while following our governor’s “Stay Home, Stay Safe” executive order, the recommendations of other health care professionals and the CDC. While we remain closed for in-person meetings, we continue to meet with families via phone, and video conferencing. Utilizing a phased approach we will begin having in-person client meetings starting on June 15, 2020. (And even LifePlan workshops.)

can be a rewarding and joyful experience. But sometimes, you may feel overwhelmed. At this workshop, you can learn how the Program of All-inclusive Care for the Elderly (PACE) can provide the support you need. The program allows your loved one to live at home, as long as possible.
Services provided may include:
Primary care, social services, home care services, prescription medicine, nutrition services, medical specialists, day center & clinic, and transportation. If you qualify, there is no cost.
