Subject: Important Notice for All CT BHP Providers: Ownership Changes

Dear Provider, 

 

Per the Important Message (IM) disseminated on February 18, 2025, from the Department of Social Services (DSS), please read the following information pertaining to any ownership changes to your practice that would impact your participation in the Medicaid program.

What you need to know


  • Notification Requirement: Providers must inform the Department of Social Services (DSS) about any changes in ownership, such as board changes, stock sales, practice sales, and alterations in ownership of 5% or more, before they occur.


  • Enrollment Compliance: Any ownership change demands a new provider enrollment application, effective from the date of the sale or change.


  • Documentation Needs: A notarized letter from previous owners is required, detailing the sale and the end date of their enrollment.


  • Non-transferability of IDs: Medicaid IDs (AVRS IDs) cannot be transferred to new ownership as part of the sale.


Inform DSS timely of any ownership changes prior to change occurring / date of sale.

Action steps



  • Submit New Enrollment Application: This must be completed and take effect from the date of sale or stock exchange.


  • Prepare Required Documentation: A notarized letter from the outgoing owners must be submitted on company letterhead.


  • Obtain New NPI: New ownership must apply for a new National Provider Identifier (NPI).



  • Specific Steps for Long-Term Care Facilities: These providers should maintain the same Medicaid ID and call the Provider Assistance Center at 1-800-842-8440 for a new re-enrollment Application Tracking Number (ATN).


Failure to adhere to the above steps when ownership changes occur could result in claims recoupment and denial, as well as liability issues for the previous owners for any fraud, abuse or tax issues that occur.
