Expected Delivery Date Required for Durable Medical Equipment Authorization

Dear Provider,

 

Moving forward for all Durable Medical Equipment (DME) authorization requests, it is the responsibility of the rendering DME supplier to provide eviCore with an expected delivery date of the DME at the time of submission for an authorization. The delivery date is needed so that eviCore can follow up with the member to ensure timely delivery of the equipment. Our DME prior authorization form has been updated to include the question, “expected delivery date”.
