Sports Physicals
MSHSAA Bylaw 3.8.1 and 4.5.4 now allows a physical exam certified by a medical professional (MD/DO/ARNP/PA/Chiropractor) to be valid for a  duration of two years from date of issue,  unless a physician specifically notes on the PPE that the physical is only valid for one year.

While the MSHSAA PPE Form is now valid for a two year period from the date of the physical examination,  all other pre-participation documents on Privit must still be completed/updated annually for every student athlete.

  • Medical History
  • Parent Permission-Insurance Verification

Student-athletes are not allowed to participate in tryouts or contests until all forms are completed and submitted correctly to the Privit website by  Friday, August 9, 2019 .

NOTE: It is imperative to use the Parent/Guardian instructions for proper submission. No paper forms will be accepted by the STA front offices or by coaches.

For questions while completing online submission, please contact PRIVIT Customer Service via  or by calling  844-234-4357

For questions about physicals and submission, please contact Lisa Gross at .

It is required that all athletes   must have a baseline test on file via the XLNTbrain website prior to the start of tryouts.  Tests must be repeated annually by all athletes.  Baseline tests are easily completed at home (recommended), by your daughter, and take approximately 40 minutes. Please  click here  for Athlete/Guardian Instructions on how to register and complete the test.   Parental enforcement of this requirement helps prevent athlete ineligibility.   Baseline tests are due Friday, August 9th.
