Dear Parents/Guardians,

As we prepare for the upcoming school year, we would like to remind all families that FASRI continues to use Magnus Health SMR (Student Medical Record) to manage Back-to-School Registration, Health Forms, and field trip permission forms.

Magnus Health provides families with secure, continuous access to their child's records and allows you to update information whenever necessary. Families can log in using their Magnus Health account credentials.

All Back-to-School Registration and required Health Forms must be reviewed, updated, and submitted through Magnus Health each year. This includes, but is not limited to:

• Emergency contact information
• Annual physical examination records
• Immunization records
• Medication administration forms
• Allergy, asthma, diabetes, seizure, or other medical action plans

Completion of all required forms is mandatory for every student prior to the start of the school year and before participating in any school-sponsored activities, including field trips.

Please log in to your Magnus Health account and complete all required updates for each enrolled child no later than August 1, 2026.

Accessing Your Magnus Health Account

You may access your Magnus Health account by:

• Visit https://secure.magnushealthportal.com and log in with your username and password.
• Visiting the Families section of the FASRI website and selecting the Magnus Health link.
• Download the "Magnus Health" mobile app from the Apple App Store or Google Play Store.



If you have forgotten your password, please use the "Forgot Password" option on the Magnus Health login page.

Please note:

All Back-to-School Registration Forms and Health Forms must be completed by August 1, 2026.

Health Forms must be updated annually on a rolling 12-month basis and whenever there is a change in your student’s health status.

Returning families: depending on the date your previous forms were submitted, updated documentation may or may not be required. 

Your Magnus Health account will reflect all required forms and applicable due dates for your student.
