What is CHADIS?
CHADIS a Questionnaire & Patient Engagement Solution that includes Clinical Process Support and incorporates Bright Futures guidelines into their process. CHADIS offers hundreds of screening, diagnostic, data collection tools and questionnaires including tools in every category advised by Bright Futures. CHADIS It automatically assigns, requests, collects, scores, interprets, and graphs patient data, which makes it different from other solutions.

Pricing:
- We have received special pricing for our group initially at $1000 a license per provider.
- If we achieve 500 providers in the program, then the renewal rate will go down to $900.
- We have a 3 year rate guarantee, so the pricing will remain stable for the next couple of year.

Reach out to CHADIS Account Rep at https://www.site.chadis.com/contact-form and identify yourself as being a TCCN member.
- CHADIS will send you a proposal
- You will need to sign a Business Associate Agreement with CHADIS
- You will need to sign a licensing agreement with CHADIS
- Pay deposit or whole based on proposal
- Account set up by CHADIS
- CHADIS Account team reaches out to practice about on boarding process
- CHADIS conducts training
- Practice goes live
5. CHADIS Account Management will work with you to on-board your practice including training and ongoing support.
My practice already uses CHADIS – How do I get the discount?
1. Email Barbara Douglas and let her know you are an existing member.
2. Barbara will notify CHADIS and then in your account they will notate you are with TCCN and when you renew – you will receive the discount.
