  2. Click on Forms (in the menu).
  3. Go to the Health Forms
  4. Complete each required form — including the emergency medical form with contact/health history, permissions, etc.
  5. Submit the form (make sure to click “Submit”).
  6.

Thank you for completing this as soon as possible — having accurate emergency medical info is critical for your child’s well‑being at school.

Conestoga 2025-2026 Online Directory

The Conestoga Directory is now available online! To access the directory, you must register, sign-in and confirm your contact information at https://conestogahs.membershiptoolkit.com. Even if you had access last year, you still must log in and register for the 2025-2026 Directory. You will also need to complete a ‘purchase’ of $0 to access the directory, as this ensures that only our community can access this information.

The CHS Directory is ‘opt-out’, so your information is included in the directory unless you indicate that you don’t want it to be. You should log in with your email address and confirm your Family Information. Once you’ve done that, you will choose Directory and Publish Preferences, where you can determine what information you’d like others to see and what you don’t want shared. If you don’t take this action, your information will appear in the directory.

Information in the CHS Directory is safe and secure and will not be shared or sold to third parties by the School District or by Membership Toolkit. Each person using the Directory must agree to a Privacy Statement.
