Transportation Survey
The Survival Coalition needs your help!  They are in need of as many people with disabilities and their families to tell us how transportation (or a lack of transportation) effects your life.  
  • Can you get where you need or want to go on your schedule?
  • Do you rely on family members, public transportation, or other options to get to your job and other places in the community?
Answers to these and other questions will help the Survival Coalition inform policy makers and work on ways to improve transportation for people with disabilities, older adults, and their families.  
 
The deadline to take the survey is July 30th, 2018.

IRIS Advisory Committee Meeting - Open to the Public
DHS has developed an IRIS Advisory Committee.  The purpose of this committee is to advise DHS with respect to any contemplated rule making.  If you have questions or concerns about IRIS and how it is impacting you or your loved ones, there is an opportunity for public comment at these meetings.  Anyone interested in speaking will be limited to 5 minutes. The next meeting is scheduled for:

The purpose of this meeting is to conduct the governmental business outlined in the above agenda. The IRIS Advisory Committee is created by DHS to advise on matters that directly concern the IRIS Program. The Committee is expected to be a provide input, guidance, and community feedback to DHS to enable them to administer the IRIS Program within the allowances of the waiver and to ensure that it is a cost-effective and sustainable Long Term Care program. The establishment of the committee is referenced in the Wisconsin 1915 (c) Home and Community-Based Services Waiver WI.0484.R02 as authorized by the Centers for Medicare & Medicaid Services More information about the Committee is available at https://www.dhs.wisconsin.gov/iris/iris-advisory-committee-charter.pdf.

MSCR Adaptive End of Summer Celebration
MSCR Adaptive recreation is hosting an End of Summer trip to watch the Madison Mallards take on the Lakeshore Chinooks.  
Thursday August 2, 2018 
Game starts at 7 pm 
Ticket Cost - $20.00 
Tickets include bus transportation, a ticket within the TDS Triple Play Club (including all you can eat food and drink), and a Mallards Bobblehead.
 
Transportation: MSCR will have two meet up locations Memorial High School at 5:00 pm (Mineral Point Rd entrance) and MSCR East at 5:30 pm (Cottage Grove Rd). Or participants can meet at Warner park at 6pm.
 
TDS Triple Play Club ticket : All-You-Can-Eat Ball Park Food including burgers, brats, hotdogs, chicken breast, potato salad, coleslaw, and chips. Bottomless Soda and Water.

Taking the first steps toward future planning is important.  And we are here to help.

Over the last number of weeks, we have led you through six key worksheets to help you gather basic information to ease you into the future planning process.  These six worksheets cover the six key areas of future planning.  If you have run into questions along the way, now is the time to review your work.
